Address is/was 132 W. 8th Ave. No zip code. Extension distributed by National Enterprises, Inc of Fargo,ND. Cannot locate any info on either company. Believe item mfg in the 50's. "Designed to fit all cars equipped with brake pedal only." (Part of ad on outside of box.)
